Care partners receive personalized guidance, practical strategies, and tools they can use at home. Home visits are also available to assess safety, routines, and behaviors, and to develop tailored safety and emergency plans.

ACCA’s Adult Day Health Program provides a safe, supportive daytime setting for adults who need medical monitoring, personal care, and social engagement.
